INSTRUCTIONS: APPLICATION/REMOVAL OF GUARDIAN (PC-500)

“Application/Removal of Guardian PC-500 Step 1”

In the first box, list the name, address, zip code, and telephone for the minor. List the child’s date of birth in the adjacent box.

“Application/Removal of Guardian PC-500 Step 2”

Fill out the second section if the minor belongs to a tribe or reservation.

“Application/Removal of Guardian PC-500 Step 3”

In the next section, provide the name, address, zip code, telephone, and legal status of petitioner. If petitioner is an adult relative, provide the date of birth. If the petitioner is counsel for the minor, list the juris number.

The next section provides names, addresses, zip codes, and phone numbers for respondents to be removed as a guardian. If the respondent is a parent, provide their date of birth. Provide their relationship to the minor.

The next section requires all contact information for other persons with guardianship rights. List their relationship to the minor child in the appropriate box on the right.

Fill out the next two sections if the whereabouts of the respondents are unknown. List their last-known address(es) and provide efforts that have been taken to contact the respondent. Check the boxes only if they apply.

On the second page of the form, check all boxes that apply. If a checked box asks for an attachment, attach the appropriate form. Sign the bottom of page 2 and move onto page 3.

The third page is for the proposed guardian(s). Sign the appropriate line and provide your name, address, telephone number, and date of birth in the appropriate sections.
